Job Description
We are seeking a detail‐oriented and knowledgeable MDS Nurse to join our long‐term care team, responsible for coordinating and completing the Minimum Data Set (MDS) process, ensuring accurate assessment, documentation, and care planning.

Requirements

  • Active Ohio RN or LPN license in good standing.
  • Experience in long‐term care or skilled nursing facility (SNF).
  • Working knowledge of MDS 3.0, care planning, and regulatory guidelines.
  • Strong attention to detail and documentation accuracy.
  • Excellent communication, organizational, and teamwork skills.
  • Proficiency with electronic medical records (EMR) systems.
