Job Description
We are seeking an MDS Nurse to join our clinical team at Anoka Homestead Rehab & Living Center. The MDS Nurse is responsible for planning and organizing the MDS/RAI process in coordination with the RAI/PPS Coordinator.

Requirements

  • Licensure: Active and unencumbered license as a Licensed Practical Nurse (LPN) or Registered Nurse (RN)
  • Experience: Minimum of two (2) years of nursing experience in a skilled nursing facility (SNF)
  • Certifications: Resident Assessment Coordinator – Certified (RAC-CT) certification required or must be obtained within six (6) months of hire
