Job Description
The Senior Medical Case Manager will help individuals navigate access to community services and other resources, and will work with diverse populations to enhance health outcomes.

Requirements

  • B.A. or B.S. with at least one (1) year of HIV or other chronic illness related field experience, preferred
  • Associate degree and three (3) years of such experience, or five (5) years of such experience
  • 2-3 years of case management related experience working in a community–based setting
  • HIV/AIDS/STI knowledge including prevention, infection, and treatment
  • Knowledge of healthcare terminology
  • Basic computer skills for word processing and spreadsheet use
  • Knowledge of AIRS and eCW strongly preferred
  • Good communication skills
  • Ability to work in a culturally diverse environment
  • Lived experience preferred
  • Bilingual – Spanish a plus
