What You Will Do
Responsibilities include completing Care Coordination Assessments, collaborating with the Interdisciplinary Team, ensuring discharge plans are initiated, and providing psycho-education to nursing and medical staff.
Why It Might Be a Fit
The ideal candidate will have a Master's Degree in Counseling, Psychology, Social Work, or a Human Services related field, and three years of experience in assessment, admission, and crisis intervention with Behavioral Health and/or Chemical Dependency patients in an acute care/medical setting.

- Masters Degree in Counseling, Psychology, Social Work, or a Human Services related field
- Psy. D. is also acceptable
- Three years of assessment, admission, and crisis intervention experience with Behavioral Health and/or Chemical Dependency patients in an acute care/medical setting
