Medical Social Worker

As a Medical Social Worker with Caringhand Home Health Solutions, Inc., you will shape how we care for patients in their homes, mentor a team of social workers, and help set the standard for compassionate, high-quality service across our organization.

What You Will Do

Lead daily operations of our social services team, including policies, procedures, and care coordination with other clinical providers. Recruit, onboard, and mentor social workers—providing clinical supervision, observing practice, and supporting their professional growth.

Requirements

• A Master of Social Work (MSW) from an accredited program
• An active California license as an LCSW
• At least 3 years of management experience, preferably in home care
• Strong knowledge of healthcare regulations, ACHC standards, and business operations
• Excellent communication skills and the ability to build trust with patients, families, staff, and the community
• A self-directed, organized, and flexible approach—you take initiative and work well with others
