Medical Social Worker

As a Medical Social Worker, you will lead daily operations of our social services team, mentor social workers, and oversee assessment, planning, and evaluation of social services care.

Responsibilities include recruiting, onboarding, and mentoring social workers, building strong knowledge of community resources, and contributing to quality improvement initiatives.

Requirements

• Master of Social Work (MSW) from an accredited program
• Active California license as an LCSW
• At least 3 years of management experience, preferably in home care
• Strong knowledge of healthcare regulations, ACHC standards, and business operations
• Excellent communication skills and the ability to build trust with patients, families, staff, and the community
• Self-directed, organized, and flexible approach
