Memory Care Support Partner

As a Memory Care Support Partner, you will be responsible for creating human connections with residents during their most vulnerable moments. You will discover residents' interests, abilities, and what makes each resident who they are, and collaborate to create appropriate engagement plans. You will also be responsible for setting the stage for a successful day, from preparing the dining room for breakfast to setting up the library for an event. You will partner with residents to achieve all facets of daily life, from the moment they wake to the moment they go to sleep.

What You Will Do

Your main day-to-day responsibilities will include documenting resident care and changes in condition, assisting with orientation of team members, and providing care and support to residents. You will also be responsible for creating human connections with residents, discovering their interests and abilities, and collaborating to create appropriate engagement plans.

Why It Might Be a Fit

This role requires a positive and energetic attitude, excellent communication skills, and the ability to work with residents who may have dementia or other cognitive impairments. You will need to be able to stand, walk, bend, kneel, and stoop all day, and frequently lift and/or move items up to 50 pounds. You will also need to be able to assist residents with sitting, standing, and walking, as well as assisting persons after a fall.

Requirements

• High school diploma or equivalent
• Ability to read, write, understand, and communicate in English
• Positive and energetic attitude
• Computer skills to print documents and enter information about residents
• Ability to stand, walk, bend, kneel, and stoop all day
• Ability to frequently lift and/or move items up to 50 pounds
• Ability to assist residents with sitting, standing, and walking, as well as assisting persons after a fall
• Criminally cleared
