Job Description
Make a Real Impact as a Mental Health Clinician on CCC's Intensive Case Management Team, providing clinical care to clients navigating severe and persistent mental illness, addiction, homelessness, and complex medical needs. Conduct mental health assessments, provide counseling, case management, crisis response, and escort clients to appointments. Collaborate with a multidisciplinary team to deliver accountable, coordinated, and client-centered care.

Requirements

  • Master's degree in a human services field
  • At least one year of mental health experience
  • QMHP or QMHP-R certification
  • LPC-A or LPC certification
  • CSWA or LCSW certification
  • CADC-R certification
  • Ability to complete thorough histories, assessments, mental status exams, and treatment plans
  • Strong communication skills and the ability to work within a multidisciplinary team
  • Reliable transportation for outreach
