Role Overview

The Brockton Multi Service Center Adult Community Clinical Services (ACCS) Integrated Team (IT) Program is seeking a dynamic candidate to serve as a Community Outreach Worker (MHW II) within one of our Adult Community Clinical Services (ACCS) teams. The ACCS MHW II will provide trauma-informed, person-centered community-based interventions that target skill development to assigned caseload of community clients.

What You Will Do

The selected candidate will serve as a member on an ACCS Integrated team by attending regularly scheduled meetings, providing updates on person served’s progress toward goals and any barriers to progress, and assisting in the development of Community Service Plans (CSPs).

The ideal candidate will have experience working with people with chronic/persistent mental illness and/or substance use disorders, and will be able to provide competency-based, evidence-based, and best practice interventions.

Requirements

  • Possession of a current and valid Class D Motor Vehicle Operator's license
  • At least one year of full-time experience in providing direct care services to individuals with a mental illness or physical disabilities
  • An Associate’s degree or higher in psychology, human services, or a closely related field may be substituted for the required experience
  • Certification in Cardiopulmonary Resuscitation (CPR), first aid, and Medication Administration Program (MAP)
