Job Description
UT Southwestern is seeking a MSW LMSW SW Care Coordinator to join their Care Coordination Department. The successful candidate will educate the healthcare team and physicians about psychosocial issues and strategies to address them. They will also apply advanced practice skills in assessment, treatment, planning, implementation, and evaluation of patient care plans.

Requirements

  • LMSW license in the state of Texas
  • 2 years of hospital experience (preferred)
  • Ability to work collaboratively with multidisciplinary teams
