Job Description
The Navigator II role at Community Bridges, Inc. involves enhancing the likelihood of success for clients, identifying and reducing barriers, and connecting them with needed services and resources. The role requires conducting outreach, engagement, case management, and wrap-around supportive services.

Requirements

  • Highschool diploma or GED
  • Minimum of six months of recovery from substance use and/or mental health disorders
  • 1-3 years of full-time Health Care related work experience
  • Minimum of ninety (90) days experience in a Navigator or Peer Support Specialist role
  • Current AZ Driver's license
  • Clear 39-month motor vehicle record
  • Arizona Fingerprint Clearance card (Program Specific)
