What You Will Do
Provide care management, care coordination, individual and family support, hospital transition planning, and health promotion services to Behavioral Health Home enrollees and Comprehensive Services clients.

Requirements
- LPN/RN
- vaccinated against influenza annually during flu season and against COVID-19 prior to start date
