Role Overview

The Nurse Navigator for Classical Hematology plays a vital role in improving referral efficiency, patient access, and care coordination for an expanding, nationally recognized tertiary referral clinic.

What You Will Do

Assesses for any barriers to care through the clinical continuum to address patient, care giver or family needs. Provides education, resources and referrals to internal and external resources based off of assessment need.

Requirements

  • Graduate of an approved/accredited nursing program
  • 3 years of clinical nursing experience
  • RN) REGISTERED NURSE Licensure as Registered Nurse of Board of Nurse Examiners for State of Texas
  • BLS) BASIC LIFE SUPPORT - current Basic Life Support (BLS) certification
