Role OverviewAs a full-time tele visit NP, you'll conduct remote assessments for home health patients, act on predictive health signals in real time, and coach care teams through virtual touchpoints to drive earlier interventions and reduce preventable hospitalizations. This role is hybrid — you'll spend three days a week seeing patients in their homes and two days a week delivering care via telehealth. You'll interpret trend-based insights from the Circadia platform, prioritize high-risk patients, and drive timely interventions to prevent decline and unnecessary ED visits.
What You Will Do
Conduct initial and follow-up telehealth assessments on flagged patients; initiate/adjust plans of care; coordinate with referring and primary care providers—all via secure video/audio platform. Complete 25+ tele visits per day for designated home health patients in the SoCal/OC area, focusing on new admissions, high-acuity flagged patients, and Circadia alerts. Complete documentation/charts within the same day.

Requirements
- Certified and licensed Nurse Practitioner in CA
- ANCC (or equivalent) in Adult/Family/Geriatric/Acute
- Eligible for Medicare participation
- 2+ years NP experience with complex adult populations in home health or post-acute settings
- Comfortable conducting telehealth visits and navigating virtual care platforms
