Nurse Practitioner - Palliative Care

We are seeking a qualified Nurse Practitioner to service our patients throughout the Charleston area. As a Provider, you play a critical role in delivering exceptional patient care across various settings, including homes, facilities, and telehealth.

Evaluate high-risk patients, develop individualized care plans, coordinate care activities, and provide care management to improve patient outcomes.

Requirements

• Nurse Practitioner (NP), Physician Assistant (PA), or Physician with a valid license to practice in the state of employment required
• Demonstrated experience in providing patient care, preferably in a home health, clinic, facility, telehealth, or specialty care setting
• Strong communication, collaboration, and interpersonal skills, with the ability to work effectively within an interdisciplinary team
• Ability to evaluate high-risk patients, develop care plans, and coordinate care activities to meet patient needs and improve outcomes
• Commitment to delivering compassionate, patient-centered care and promoting a culture of excellence in healthcare delivery
• Strong organizational and time management skills, with the ability to multitask and prioritize responsibilities effectively
• Ability to read and communicate effectively
• Strong written and verbal skills
• Basic computer knowledge
• Ability to manage and demonstrate effective leadership skills
• Ability to foster a cooperative work environment
• Team player with ability to manage multiple responsibilities and demonstrate sound judgment
• Must be able to work flexible hours and travel between offices, facilities, etc.
• Must be a licensed driver with an automobile that is insured in accordance with state and/or organizational requirements and is in good working order
