Job Description
The Advanced Practice Provider (APP) collaborates with a multidisciplinary healthcare team and is service aligned, managing a defined patient population in the outpatient setting. This role emphasizes general, episodic, and preventive care while delivering patient-centered services.

Requirements

  • Obtains and documents a health history
  • Provides healthcare services for primary, acute and complex care of patients and manages patients' overall care
  • Orders, performs, interprets and collects data using appropriate assessment techniques
  • Forms differential diagnoses and treats acute and chronic conditions
  • Prescribes therapeutic interventions for both pharmacologic, non-pharmacologic
