Role OverviewPHP is recruiting a Nurse Practitioner to provide high-quality primary care to seniors and other residents in a variety of healthcare settings, including skilled nursing facilities, assisted living, and independent living communities, as well as via telehealth.
What You Will Do
Assess, diagnose, and manage chronic and acute conditions, provide health education, develop treatment plans, and collaborate with local and regional operations and clinical leadership to achieve goals.

Requirements
- Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate
- National certification of AANP or ANCC
- Active, unrestricted state license
- Active DEA license
- US work authorization
- 2+ years clinical experience as a Nurse or Nurse Practitioner in geriatric, adult or family practice setting
