Perform comprehensive physical exams, diagnose and document chronic disease, understand and incorporate palliative and end-of-life principles into care plans, and work collaboratively with physicians and other healthcare professionals.

Requirements
- Graduate of an accredited Adult or Family Nurse Practitioner Master’s Degree program
- Minimum 1 year clinical experience as a Nurse Practitioner in geriatric, adult or family practice setting
- Current and unrestricted, RN and NP licensure in state of practice
- Board certification by AANP or ANCC, in Adult, Family Practice and Geriatrics or Adult-Gerontology Primary Care
- Current CPR certification
