Role Overview

The Nurse Practitioner provides hospice services, complementing attending physician care, and collaborating with the Interdisciplinary Group (IDG) to ensure patient needs are met. They evaluate hospice eligibility, perform face-to-face visits, and assure patients receive appropriate symptom control. The Nurse Practitioner works in a team approach with the IDG, participating in plan of care development and managing oversight of medications and treatment.

What You Will Do

Main responsibilities include providing direct medical services, attending IDG conferences, participating in performance improvement programs, and reviewing and developing protocols for treatment. The Nurse Practitioner also documents care provided, maintains current knowledge of the latest research and trends in hospice care, and demonstrates knowledge in communications and counseling patient/family in dealing with end-of-life issues.

Requirements

  • Completed an accredited Nurse Practitioner Program
  • Current nursing licensure in State
  • Experience with end of life care
  • Maintains controlled substances registration with state and federal authorities, if applicable
  • Not excluded from the Medicare program
