Role Overview

The Nurse Practitioner provides hospice services, including evaluating hospice eligibility, performing face-to-face visits, and ensuring patients receive appropriate measures to control uncomfortable symptoms. They collaborate with the IDG to ensure patient needs are met and support clinical and administrative staff in providing compassionate quality care.

Requirements

  • Completed an accredited Nurse Practitioner Program
  • Current nursing licensure in State
  • Experience with end of life care
  • Maintains controlled substances registration with state and federal authorities, if applicable
  • Ability to demonstrate self-confidence and positive attitude toward self and others
  • Not excluded from the Medicare program
  • Ability to communicate well, both verbally and in writing
