Requirements
- Nurse Practitioner (NP), Physician Assistant (PA), or Physician with a valid license to practice in the state of employment required.
- Demonstrated experience in providing patient care, preferably in a home health, clinic, facility, telehealth, or specialty care setting.
- Strong communication, collaboration, and interpersonal skills, with the ability to work effectively within an interdisciplinary team.
- Ability to evaluate high-risk patients, develop care plans, and coordinate care activities to meet patient needs and improve outcomes.

- Strong organizational and time management skills, with the ability to multitask and prioritize responsibilities effectively.
