Role OverviewWe are seeking a qualified Nurse Practitioner to service patients in the Athens, Georgia area. As a Provider, you will play a critical role in delivering exceptional patient care in patient homes and facility settings. You will be an integral part of the care team, collaborating with multiple healthcare professionals to ensure comprehensive and effective patient management.
What You Will Do
Evaluate high-risk patients, coordinate care plans, and provide care management to improve patient outcomes. Perform visits in your designated service area, communicate with case managers and other team members, and document care management activities accurately.

Requirements
- Nurse Practitioner (NP), Physician Assistant (PA), or Physician with a valid license to practice in the state of employment
- Demonstrated experience in providing patient care, preferably in a home health, clinic, facility, telehealth, or specialty care setting
- Strong communication, collaboration, and interpersonal skills
- Ability to evaluate high-risk patients, develop care plans, and coordinate care activities
- Commitment to delivering compassionate, patient-centered care
- Strong organizational and time management skills
- Ability to read and communicate effectively
- Basic computer knowledge
