Job Description
Monogram Health is seeking a skilled Nurse Practitioner or Physician Assistant to join their team. The ideal candidate will have experience in direct patient care and be able to work autonomously. The role involves delivering personalized medical care to patients with multiple chronic conditions, conducting assessments, and coordinating care with primary care providers and specialists.

Requirements

  • Active and unrestricted Registered Nurse and Nurse Practitioner or Physician Assistant license
  • Board certified for appropriate licensure (NP: ANCC/AANP; PA: NCCPA)
  • Current and unrestricted DEA certificate
  • 2 or more years of direct patient care required
  • Managed Care/IPA/Health Plan experience
  • Experience conducting annual wellness visits or similar comprehensive visits virtually or in the home
