Job Description
Monogram Health is seeking Advanced Practice Providers (Nurse Practitioners or Physician Assistants) to deliver high-quality in-home care to patients with multiple chronic conditions. The role involves establishing long-term patient relationships, providing continuity of care, and participating in an integrated multidisciplinary care team.

Requirements

  • Active and unrestricted RN and NP license, or Physician Assistant license
  • Board certification (NP: ANCC/AANP | PA: NCCPA)
  • Current and unrestricted DEA certificate, or eligibility to obtain
  • Minimum of two (2) years of direct patient care experience
  • Field‐based role requiring travel to patient homes, reliable transportation, valid driver’s license, and auto insurance
