Nurse Practitioner/Physician Assistant

As an Advanced Practitioner at Community Care Internal Medicine – Delmar, you will provide adult primary care in a collaborative, team-based environment alongside board-certified internists and experienced Advanced Practice Providers.

What You Will Do

Conduct comprehensive evaluations for adult patients and manage acute and chronic conditions, order and interpret diagnostic studies, prescribe medications, and coordinate specialty referrals.

Requirements

• License and active registration as a physician assistant or registered nurse/nurse practitioner in New York State.
• Bachelor's or equivalent degree in physician assistant studies and successful completion of physician assistant training program approved by the NYS Department of Education or accredited by the Accreditation Review Commission on Education for the Physician Assistant.
• Attainment of passing grade on the Physician Assistant National Certifying Examination administered by the National Commission on Certification of Physician Assistants.
• Current certification by National Commission on Certification of Physician Assistants.
• At least one year of experience as a physician assistant in ambulatory health care.
• Current CPR certification.
• Successful completion of and graduation from a nurse practitioner education program acceptable to NYSED.
• Current certification as a nurse practitioner by the American Academy of Nurse Practitioners Certification Board (AANPCB) or the American Nurses Credentialing Center (ANCC).
• Successful completion of pharmacology coursework acceptable to NYSED.
• Three years of professional nursing experience in an ambulatory setting preferred.
• At least one year of experience as a nurse practitioner in ambulatory health care.
• Current CPR certification.
