Role OverviewAs an Advanced Practitioner in the Weight Management and Metabolic Care program, you will provide evidence-based care for adult patients with obesity, Type 2 diabetes, and related metabolic conditions. You will work within a highly collaborative, multidisciplinary team to deliver personalized treatment plans that integrate lifestyle modification, medical therapy, and ongoing support.
What You Will Do
Conduct comprehensive evaluations, develop and manage individualized treatment plans, monitor patient progress, provide education on nutrition and behavior change strategies, and collaborate with a multidisciplinary team.

Requirements
- License and active registration as a physician assistant or registered nurse/nurse practitioner in New York State.
- Bachelor's or equivalent degree in physician assistant studies (for Physician Assistant role)
- Successful completion of a nurse practitioner education program (for Nurse Practitioner role)
- Current certification as a nurse practitioner or physician assistant
- At least one year of experience as a physician assistant or nurse practitioner in ambulatory healthcare
- Current CPR certification
