Role OverviewWe are seeking a compassionate and experienced Nurse Practitioner (NP) to provide high-quality healthcare services in a Skilled Nursing Facility (SNF) in Plainfield, IN. The Nurse Practitioner (NP) will work closely with physicians, nursing staff, and other healthcare professionals to assess, diagnose, and treat patients while ensuring continuity of care.
What You Will Do
Conduct comprehensive patient assessments, develop individualized treatment plans, diagnose and manage acute and chronic conditions, educate patients and families on disease management and preventive care, and maintain accurate and timely documentation.

Requirements
- Active Nurse Practitioner (NP) license in the state of Indiana
- Experience in a Skilled Nursing Facility (SNF), long-term care, or geriatric setting preferred
