Role OverviewProvide evaluative, consultative, and treatment interventions to home health patients referred by a licensed physician. Collaborate with the interdisciplinary team to develop and manage the patient care plan.

Assess, monitor, and document patient progress; help maintain continuity of care; recognize and respond to priorities in patient care; and utilize accepted company teaching materials in patient/family education.

Requirements
- Valid and unrestricted professional license in state of practice
- Graduate of ACOTE accredited school
- Foreign-trained program must be deemed substantially equivalent to an Occupational Therapist entry level education in the U.S. by a credentials evaluation organization approved by the AOTA
