Role OverviewProvide evaluative, consultative, and treatment interventions to home health patients referred by a licensed physician. Collaborate with the interdisciplinary team to develop and manage the patient care plan.
What You Will Do
Assess, monitor, and document patient progress; develop and integrate patient care plans; provide patient education; and complete documentation timely and accurately.

Requirements
- Valid and unrestricted professional license in state of practice
- Graduate of ACOTE accredited school
- Foreign-trained occupational therapists must have their program deemed substantially equivalent to an Occupational Therapist entry level education in the U.S. by a credentials evaluation organization approved by the AOTA
