Role OverviewProvide evaluative, consultative, and treatment interventions to all home health patients referred by a licensed physician in their place of residence.
What You Will Do
Assess, monitor, document, and report patient progress; collaborate with the interdisciplinary team to develop and manage patient care plans; and provide patient education.

Requirements
- Valid and unrestricted professional license in state of practice
- Graduate of ACOTE accredited school
- Foreign-trained occupational therapists must have their program deemed substantially equivalent to an Occupational Therapist entry-level education in the U.S. by a credentials evaluation organization approved by the AOTA
