Role OverviewAs a Child Focus OhioRISE team member, you will serve as the point of contact for assigned OhioRISE youth, providing comprehensive care coordination for all youth and family service needs. You will work collaboratively across systems, including education, behavioral health, juvenile justice, and developmental disabilities. Your goal is to strengthen the community by investing in the youth and their families.
What You Will Do
Your main responsibilities will include structured service planning and care coordination, conducting care coordination activities, completing comprehensive assessments, convening and facilitating child and family teams, and performing referrals and linkages to appropriate services and supports.

Requirements
- High School Diploma / GED
- 3 years of relevant experience
- Bachelor's Degree in Social Work, Psychology, Criminal Justice, Human Services
- 2 years of experience
- Master's Degree in Social Work, Counseling, Psychology, Criminal Justice Human Services
- 1 year of experience
