Social workers provide brief crisis counseling and therapeutic intervention with particular focus on frequent utilizers of the healthcare system. Provides therapeutic intervention to facilitate complex discharge planning by completing psychosocial assessments and high-risk screens after independent case-finding or receiving referrals.
