Job Description
LSSI is hiring a full-time Outreach Coordinator to identify, reach out to, and engage residents at Chicago area SNF and SMHRFs, providing education and information on the program and the scope of services available post-transition.

Requirements

  • High school diploma or equivalent required
  • Background check clearance required
  • Bilingual/Spanish preferred
  • Customer service experience preferred
  • Demonstrated ability to communicate in a clear, comprehensible manner, both verbally and in writing
