Provide person-centered services to individuals in the community, assist with discharge planning, and connect them to needed supports.

Requirements
- High School Diploma or GED
- 1–2 years of experience in behavioral health, substance use, corrections, or related human services field
- Knowledge of mental health, substance use, and community resource navigation
- Valid Massachusetts driver’s license and reliable vehicle
