Job Description
Community Bridges, Inc. (CBI) is seeking an Outreach Navigator to work closely with Peer Operated Center program participants to establish short-term and long-term goals, focusing on employment stabilization, housing, and income stability. The position involves monitoring participants' transition from non-POC engagement to POC enrollment and assisting them with accessing needed benefits and services.

Requirements

  • High school diploma or GED required
  • Associates Degree (or higher) in a field related to Behavioral Health preferred
  • Minimum of six months of recovery from substance use and/or mental health disorders required
  • 1-3 years of full-time Health Care related work experience preferred
