Job Description
The Outreach Navigator is a dedicated advocate and support professional in navigating the complexities of healthcare and social services who has experience with HIV, Hepatitis C, or HIV Pre-exposure Prophylaxis (PrEP) or has been affected by substance use disorder. This role focuses on enhancing the quality of life for patients by providing comprehensive support, including healthcare coordination, education, and linkage to essential services.

Requirements

  • Linking those living with HIV to treatment, follow-up on medical appointments, adherence to education, and group support
  • Works collaboratively with the Medical Case Manager and Clinical Social Worker to bring support services to the client
  • Responsible for conducting community screening events and providing referrals for prevention services
