Role OverviewThe Palliative Care Practice Manager supports the operational, administrative, and cultural success of the palliative care program. This role serves as a key operational leader responsible for developing scalable workflows, supporting interdisciplinary collaboration, optimizing patient access and care coordination, and assisting with practice growth initiatives.
What You Will Do
Provide operational oversight and coordination for the palliative care practice, support the development, implementation, and ongoing management of Principal Care Management (PCM) workflows and processes, and collaborate with leadership to operationalize new programs, initiatives, and workflow improvements.

Requirements
- Associate’s or Bachelor’s degree in healthcare administration, business, nursing, or a related field
- Minimum of 3 years of healthcare experience
- Previous experience in palliative care, hospice, home health, chronic care management, care coordination, medical practice operations, or healthcare leadership
- Experience with Electronic Health Records (EHR) and healthcare technology systems
- Experience with workflow development, process improvement, scheduling coordination, or healthcare operations
- Strong organizational, communication, and leadership skills
- Ability to manage multiple priorities in a fast-paced healthcare environment
- Knowledge of Medicare, healthcare billing workflows, and care coordination models
- Experience with telehealth workflows, care management programs, and/or PCM/CCM services
