Role Overview

The Palliative Care Social Worker provides counseling and medical social work case management to patients, their families, and caregivers. This position assesses the social and emotional factors related to the patients illness and evaluates the psycho-social response to treatment and level of adjustment.

What You Will Do

Provides outpatient psycho-social evaluation for advanced illness patient population, assesses and responds to psycho-social, financial and legal concerns, assists patient and family in accessing community resources, and documents in the electronic medical record.

This position requires a Master degree in Social Work, at least one year of experience as a social worker in a healthcare setting, and ability to work independently with a minimum of supervision.

Requirements

  • Master degree in Social Work from an accredited college/university
  • At least one (1) year of experience as a social worker in a healthcare setting
  • Ability to work independently with a minimum of supervision
  • Ability to listen and respond non-judgmentally with respect to others beliefs
  • Ability to communicate effectively verbally and in writing
  • Valid drivers license, proof of acceptable automobile insurance coverage, and reliable transportation
  • Reliable mobile phone with plan that provides good local coverage
  • Good computer skills, proficiency in Microsoft Office applications and electronic medical record systems
