Role OverviewSignify Health is seeking a Part Time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care.
What You Will Do
Conduct in-home health evaluations, including medication and medical history review, physical evaluation, and diagnostic tests if ordered by the person's health plan.

Requirements
- Active, unrestricted license(s) in coverage area(s)
- Board certification (required)
