Role OverviewPart-time social worker responsible for identifying and providing interventions for residents' social and emotional needs, and acting as a liaison between residents, families, and facility staff.
What You Will Do
Visits residents, prepares documentation, participates in resident care plan meetings, provides counseling and emotional support, and coordinates discharge planning.
