Job Description
Hilltop's mission is to create connection to build a community where everyone belongs. This role is responsible for supporting residents at Pathways Village Apartments to identify goals, needs, and resources for long term household stabilization.

Requirements

  • Become trained and/or certified in assigned Hilltop Program, curriculum or practice, and conduct duties in accordance with fidelity standards, grant requirements and program guidelines.
  • Utilizing both a person-centered and trauma-informed care approach, assist participants in creating/identifying and reaching goals aimed at long term stabilization for the household.
  • Maintain new and current participant data using program-established documentation processes, including creating and maintaining physical and electronic participant files.
