Patient Care Coordinator San Diego

Requirements

• Valid California Driver’s License and active auto insurance
• Minimum of 1 year of experience in case management, care coordination, member services, healthcare support, or customer service roles
• Demonstrated ability to navigate and document across multiple systems, platforms, and databases simultaneously
• Strong computer literacy, including proficiency with data entry, email, chat platforms, and web-based applications
• Ability to manage competing priorities, track multiple cases, and meet documentation deadlines
• Excellent organizational skills with strong attention to detail and data accuracy
• Strong communication and interpersonal skills, with the ability to translate complex information across systems and audiences
• Comfortable working in fast-paced, highly regulated healthcare environments
• Must be able to maintain reliable attendance the first 90 days of employment
