Job Description
The Patient Care Manager (PCM) is responsible for clinical oversight of all direct patient care, working through and with a variety of staff members, demonstrating the philosophy, mission, and service standards of CHOT/Providence. The position involves managing direct patient care, reviewing patient medical diagnosis, assisting in the screening and interviewing process of new employees, and maintaining compliance with regulatory and accrediting agencies.

Requirements

  • Requires a college degree in nursing
  • Requires 2+ years of hospice experience and supervisory experience
  • Experience in a Charge Nurse role will be considered to meet the supervisory requirement
  • Hospice experience and knowledge of hospice regulations strongly preferred
  • Proficient skills using a computer are required
  • Good communication skills and the ability to travel independently
