Job Description
Role Overview

The Patient Care Navigator performs as a Home and Community-Based Care Based Alternatives (HCBA) department generalist and first point of contact. The role requires extensive knowledge of related vendors operations and communicates directly with participants and facilitates appointments.

What You Will Do

Acts as initial point of contact for all department calls, provides administrative support, orders office supplies, manages emergency alert systems, and receives and delivers clinical information.

Requirements

  • Associate degree preferred
  • 3-5 years of progressively responsible experience related to health plan operations
  • Demonstrated excellent customer service and oral communication skills
  • Health plan experience with member coordination/communication and clerical management experience
  • Basic knowledge of PCs and related software
  • Accuracy, productivity, dependability, and good attendance

Benefits
