Job Description
We are seeking a Patient Navigator to join our dedicated team and support the health and well-being of our patients living with HIV. As a Patient Navigator, your responsibilities will include acting as a patient advocate, providing field-based case management and adherence support services, and assisting patients in navigating the healthcare system.

Requirements

  • High school diploma or GED required
  • Experience in social services, case management and/or entitlements work
  • Familiarity with LGBTQIA, HIV and health-related issues and community resources
