Job Description
The Heart Failure Center Patient Service Line Navigator provides coordinated care to patients and the multidisciplinary team to achieve optimal patient outcomes. They provide tailored education and support to patients and caregivers, assist with patient management, and review and manage referrals.
Requirements
Bachelors in Nursing (BSN) from an accredited School of Nursing (or must obtain BSN within 2 years of hire)
Licensure to practice as a Registered Nurse by the State of Florida
MSW degree from a CSWE accredited college or Social Work program
Master's degree in Health Administration or related field
Three years related work experience
BLS certification
ACLS certification preferred
