Job Description
The Patient Support Worker welcomes, orients, and engages all patients on-site at the health center, providing general information and direction regarding accessing and following up on their services.

Requirements

  • High School diploma
  • At least 1 year of related customer service experience in a health care setting including face to face interactions with patients
  • Direct lived experience in addiction and/or mental health recovery preferred
  • Knowledge of Connecticut community resources and organizations
  • Exhibits strong interpersonal, critical thinking, communication, and problem-solving skills
  • Demonstrates proficiency in Microsoft Office applications and other software programs
