Role OverviewAs a Pediatric Transitional Nurse, you'll play a critical role ensuring safe, successful discharges, preventing readmissions, providing interim patient coverage, and supporting continuity of care until permanent staffing is established. This role is ideal for nurses who are confident working with high-acuity patients and skilled in trach, vent, G-tube, and emergency management.
What You Will Do
Provide skilled pediatric nursing 1:1 care in patients' homes, administer medications, treatments, and therapies, support hospital-to-home transitions for high-acuity patients, and educate and support parents, families, and caregivers.

Requirements
- Active, unrestricted Registered Nurse (RN) or Licensed Vocational Nurse (LVN)/Licensed Practical Nurse (LPN) license (PA)
- Current CPR certification
- Ability to lift and assist pediatric patients as needed
- Pediatric home health experience
- Experience with Trach care and Ventilator management
- High-acuity pediatric patients
