Job Description
Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency offering a variety of different programs throughout Arizona, Oklahoma, New Mexico, Oregon, and the District of Columbia. The Continuing Care Peer provides community-based outreach, support, navigation, and care coordination for individuals experiencing behavioral health challenges, substance use, homelessness, or crisis situations.

Requirements

  • High school diploma or GED required.
  • Minimum six months of recovery from substance use and/or mental health challenges.
  • Experience in outreach, peer support, homeless services, or behavioral health preferred.
  • Driver’s license (valid and in good standing) required.
  • Clear 39-month motor vehicle record required.
  • Peer Support Specialist certification from the Oregon Health Authority required or able to be obtained within 90 days of hire.
