Requirements

  • Assist participant in connecting with their community and assist in building lasting relationships via transportation and participating in events, volunteering, and more
  • Support participants with daily living skills such as cleaning, cooking, budget management, and light household duties
  • Following care plans and helping participants achieve their personal goals
  • Encourage healthy habits, studies, hobbies, and social activities
  • Assist with personal care such as medication support, hygiene and bathing (lifting / transferring may be required)
  • Collaborate Person-Centered Care planning with participant, staff and management
  • Document shift and communication logs
